Hey there, did you buy on Steam and then cross-save with XBox?
That would have tied the episode passes and dungeon key (which count as in-game addons, applied to the Active account at the time of purchase) to Steam, but Steam got made dormant. The passes do not move or duplicate over to XBox.
The addon form is the correct way to fix this, so your issue is that the form link is not taking you to the ticket screen? Did you try clearing your cache, and trying other browsers?

That said, with respect to being unable to access the form, you can also try an incognito window to see if that lets you access it, and some players have confirmed that using a different device works as well.

Also, can you try signing out of your Bungie profile before trying to access it, clearing your browser cache, then signing in again when prompted?

Once you log in, after it redirects you to Bungie.net from the Help site, use your browser's Back feature to see if that then shows you on the Help site as logged in. This has worked for me and some others, so it might work for you.
