Going into the little girl's room gets you killed if you stay there and don't immediately leave after encountering the child. Going into the black door causes the Enslaved Woman to kill you unless you have the mouth gag. Going into the Room of Darkness always gets you killed except for in the sequence starting with 1, where you can kill the Flying Beast (but not escape the asylum).
