Hello,
When you're experiencing a reward duplication via the Prismatic Recaster and its decrypter, please re-load yourself into the Annex via the map menu to fix it.

Yeah, I did that, I think. I turned in 9. Thought..hmmm interesting. Went back to courtyard, picked up three more, fast traveled back to annex and the last 3 were chest as well.

Hi, Then I think it's just bad luck. Re-loading yourself into the Annex should fix countless of the same item(s) duplicating.
