Occasionally, my fireteam cannot launch an activity after a previous activity without having to disband and reform as a fireteam. We are often stuck looking at a flashing launch button. Noone is in menus nor are we tabbed out. Is there a fix to this or is this a known issue and we just have to continue to rejoin each other?
