Can you confirm that you didn't leave Sundered Doctrine over the course of your completion? Note that disconnections/network issues of any kind would likely count for this, and would invalidate your run as far as the mentioned triumphs are concerned. If you're sure this is the case, though, please try a new run and, if the issue occurs again, use your platform's feature to capture recent gameplay as a video to show the issue occurring. Once you have some footage, you can upload it to a public video hosting site like YouTube, and we'll be sure to take a look.
