Check your quest log for 'Choir: Eternal', you are only given one quest at a time but once you complete one as you've already met the requirements Banshee will just give you the other quests auto completed and drop the catalyst traits. If you don't have it in your quest log, can you get a few clips/screenshots showing: Banshees vendor screen, your quest log, your abandoned quest kiosk (seasonal), and Failsafe at the HELM and link to it here. The fact the triumph that is separate to that checklist is also missing means you haven't done that specific encounter.
