Leveling up Vanguard, Crucible or Factions. Go to the corresponding vendor and there is a diamond in the upper left showing your current level and progress to the next level. Once you level them up you will receive a care package via the Postmaster. It will contain the appropriate Commendation and possibly some other items such as weapons, armor, Strange Coins or Motes of Light.
