Warlock 2 tells how Errana-3 and Toland die.
My conclusions are:
Name: Class
Death
Errana-3: Sunsinger
Killed by Crota's Blade
Toland: Voidwalker
A knife made from his own ghost by Crota.
Omar: Gunslinger
Excruciating death at the hands of Heart of Crota.
Sai Mota: Bladedancer
Almost made it out, felled by Omnigul's Thralls.
Vell: Striker
Killed at the Temple by Verok.

Toland isn't explicitly said to die, just that he wants to hear the death singers song. Being very curious about it and hearing the whole song.

In Warlock 2 a captured wizard shows Errana-3 that she dies to Crota's blade and a Crota kills another guardian with a knife made from his own ghost. Not explicitly Toland but he's the only male guardian not to have a confirmed death.
