Last week I was patrolling mars with my titan when I decided to do my shoulder attack to a phalanx, I did my shoulder attack right when he decided to do his shield attack, we flought away and both me and the phalanx glitched out and got killed by the wall...

I've died so many times to those Phalanx shields. In fact I died in a similar way earlier today while fighting Crota. I was sniping Boomers as usual and I just randomly slid forward a few feet and hit a wall. It told me in the death screen it was a misadventure.
