Grenade disappears
Its happened many times - I throw a grenade in PVE, it lands on an enemy, they hunker down for the explosion, and then...nothing.

This happened to me last night in the hard raid. Went through the Venus portal to grab the relic after killing the gatekeeper. Threw a solar grenade down on all the minions. Nothing. Pissed me off. And my grenade had to recharge then.
