Account Issue or what?

Game freezes and crashes this prompts CE-34878-0 or has to be hard reset. The issue has happened in every game mode and in every destination including the tower over the last 1.5 years. I have had this happen at random intervals of game play time and ~ 80 times. Playing on Console PS4 Pro I have tried all of the following: Reinstall from disc; Reinstall from digital copy; LAN cable; Wifi 5GHz, 2.4GHz; reset router; port forwarding open 80, 443, 3478, 3470, 3480; restore licenses; login under different user on ps4; renamed PS4; notifications disabled; Bluetooth devices (used and not used); audio devices "headphones" (used and not used); external SSD (used and not used); original hard drive (used and not used); themes dynamic and static used; resolutions used auto, 720p, 1080i, 1080p, 2060p-YUV420, 2060p-RGB, RGB range used auto, full, limited; HDR on and off; deep color output on and off; system software update; boost mode on and off; enable hdmi device link on and off; enable HDCP on and off; rebuilt database; system has been initialized; internet confirmed stable and with adequate speed for up and down stream transmissions; PS Plus in good standing; Destiny 2 up-to-date current v.2.46; system internal components removed and cleaned including vents, heatsink and fan; replaced thermal paste (no overheating before or after). Never had this issue with any other game title on this system. I first started playing this game back on PS3 when it first came out and got my son into it but in this condition I can't continue. Please reach out. Thanks
