I replicated the bug with near 100% efficiency (sadly enough) in Crucible modes including iron banner, elimination, control, and mayhem. Interestingly enough, the only time the bug did not appear was when I achieved a double kill with a single grenade. After this double kill I proceeded normally chaining roaring flames grenade kills and refunding grenade energy. However the bug reappeared once I was killed and respawned.
