You hit the grenade button. You die. Grenade never launched.
Has this ever happened to you in Crucible?
You're one shot away from dying. In a last ditch effort, you hit the button to toss a grenade at an enemy team member. You die. The grenade never launched.
You're 100% positive with zero doubt you hit that button at least a second before you died.
Happens to me all the time and annoys the shit out of me. Kinda feels like I was cheated out of the possibility of a postmortem kill.
What causes this? Is it the reaction time of the console controller or the console itself?
Is it latency/lag on the XBox/Playstation service?
Or is it latency/lag on Bungies end?
Wondering what peoples thoughts are on this.

I've noticed that sometimes when I shoot my fusion rifle it will completely stop before it fires and put the weapon up like I am running, then before I can get the next shot off I've been taken out. Now I know that if you shoot you fusion rifle and you get killed before the darts land they magically dissappear, but to just shut down my shot is weird.
