What Defines a Melee Attack?

So all classes have a standard "punch" attack that counts as melee but then you have special attacks that are called your "melee ability" But not all of these melee abilities count as melee kills. Why? Titan Shoulder Charge/Elemental Strike = Melee (all three subclasses) Warlock [Elemental] Strike = Melee (all three sub-classes) Hunter Backstab = Melee BUT throwing knife and smoke do not count as melee (1 sub-class) When there is an event for say, CoE. (or a bounty that requires melee kills) Hunters are restricted to choosing one sub-class to be competitive/efficient when melee bonus points are on the line. And crucible stats can't be easily compared to another class when it comes to melee kills. So what defines a melee attack? Is it physical contact with an enemy resulting in a kill? In that case, no Warlock strikes would count as they all kill with an unseen (magic) force. So is it arbitrarily decided what melee abilities actually end up counting as melee attacks? It seems to create a balance issue.
