Where are you?
Certain places, such as raids, will not count towards grimoire. I don't know where else that fails to happen (maybe Poe) but raids is definitely one of them

Yeah I have done story 3 times and tons of bounties for story missions too- not all yellow bars. I've killed about 35,000 vandals so don't know how I'm at 90 ogres...
