Passage of Persistence requires 7 straight wins, the flawless box doesn't get marked off if you get a loss even though your card isn't flawless. Any losses prior to obtaining 7 straight wins require the card to be started over

Yeah I made that same mistake before, it's not super clear how it works. Mercy is probably your best bet, as it forgives two losses if you haven't been flawless for the week. Seems to be bugged now though so sometimes only forgives one loss.
