I had the same experience specifically at the boss when the brig shows up. Ruin my solo flawless run... Before. I thought I was dying to random traps that spawned in, but like you I was in the back big open area. Full health, then bam, dead. I assume it's a visually bug, dying to artillery that doesn't visually load in.
