Bungie wont manually issue the triumph.
Did you have any error codes/black screening during the dungeon?

No error codes, but I noticed it wasn't teleporting me out of the nightmare realm and that's when all the triumphs showed up at the bottom of the screen. I did get the flawless emblem, so that makes me feel a bit better. This d2checklist link even shows my completion, but oh well. Thanks for the reply!

The emblem comes from the triumph, and you have all the duality flawless ones unlocked. Are you sure you aren't mistaking a duality triumph with another dungeon? I can see through braytech you have the grasp of avarice flawless triumph tracked that awards the shader
