We've only had it happen once recently, so most of the time we haven't had anyone die. We used to have one person die almost every time we would ring a bell, and a few times we had two die, one time the whole team wiped but usually at least one person would be left alive. It's really frustrating when you pick up your thing and then die getting teleported. We haven't found anything that seems to cause it as far as where we stand or if we move or not. It feels random.
