I notice more with the upstairs if you pick up scanner right next to a number pillar it doesn't ping or mark it on the radar as correct. If none of the other 3 are pinging correct it's probably the one closest to where you picked it up.
I say probably but it's worked for me 100% of the time when there's no correct ping or radar mark

You just have to back away from it, and it'll work as normal when you walk back into it.

This happened to me and I had no clue, picked up scanner next to a pillar.

As far as dungeon concerns go, it's unfortunately low on the totem pole, but it's rather consistent
