Solution for Bungie:
1. Do not allow 3 or 4 stacks fight solo's
2. Do not start the game until the queue is full
3. Do not join new players during the game
4. Let the players who left the game (for example, by the error code) allow the return to join the game - the screen with the decision "YES / NO".
5. Ten times choosing NO - Treat as rage quiting - 1 week BAN for PVP activity
6. Repeat

Problem with penalties for disconnects is that often times, it's the opposing team force-disconnecting the top scorer on your team, sometimes via DDoSing. It's not really fair to punish the victim of DDoS for 'quitting' a match, since they didn't quit, they were disconnected. And the DDoS attacks are relatively short, just long enough to keep them from rejoining the match. And because D2 takes forever to load even to the title screen, let alone back to a state where you can rejoin a Comp match, the match will be half-over. More reasons Bungie need to go with dedicated servers.
