I bought a NEW copy of Destiny: The Collection for PS4 from Amazon. The DLC code inside says it is valid from 09/20/16 - 09/20/18, but the code said it was invalid. A Playstation tech support rep looked the code up and told me the code actually expired on 12/31/17. I have no way of contacting Activision or Bungie directly, so please respond. I can give you the chat and case number from Playstation, an invoice from Amazon that the purchased item was new, and a screenshot of the DLC code with the (incorrect) expiration date.

Hello, you will need to contact Amazon for a replacement code. Bungie do not produce, manage or distribute codes. The retailer is responsible for issuing working codes with new purchases.

I had the exact same issue. Bought a new copy of Destiny: The Collection from Amazon and the code does not work. I reached out to Playstation Support and they said the code was not used, but rather has expired as of Dec. 31. My card says it doesn't expire until 09/20/18 as well. They said this is an error on Bungie's part and to contact Bungie.
