When throwing Threaded Spike, grappling to an enemy for a grapple melee, and the spike starts returning, the catch animation prioritizes over the melee input. This causes you to auto catch instead of hitting the enemy. It interrupts combos, wastes the grapple, and kills flow in PvP. It is 100 percent consistent when timings overlap.
