Alright, to get the code, you need to answer these three questions correctly.
1. What are full names of the members of Fireteam Osiris?
2. Other than the Librarian, the Didact had another possible love interest as well; what is her name?
3. What event unintentionally sparked the events that would lead to the creation of the Covenant?

3. After Halo was fired, and Mendicant Bias was entombed on the Ark, he was able to break off a shard of himself, and attempted to pilot a Keyship to Earth. Unfortunately, the ship crashed on Janjur Qom. This would lead to the Reformist San'Shyuum to steal the Keyship, leading to the discovery of the Sangheili. With the aid of the Keyship though, the Reformists would put the Sangheili in a position where they'd need to sign the Writ of Union to survive.

An unknown complication occurred however and the keyship crashed on the surface of the San'Shyuum home world. Later the San'Shyuum discovered it and began to worship it. They wanted more artifacts and this led to the war that led to the foundation of the Covenant. Now if you ask me... I think I win.

Eons pass and the Covenant, formed out of a union between the former enemies of elites and prophets (autoCorrect won't let me type their full names) mythologized the Forerunners as gods who used the "sacred rings" to ascend to divinity, essentially building their whole religion on a falsehood.

The Forerunners activating the Halo installations, you can't have a religion (The Covenant) without this event happening and trying to "fulfill the Great Journey" the Forerunners "took". I doubt the Forerunners intended for a future alien covenant to form around their desperate attempt at keeping the flood at bay. (Referred to as the Age of Abandonment)

I'm not sure what unintentionally started that, I assume it was mentioned in Broken Circle but I haven't read it :( Edit: There was a skirmish on a planet between the elites and prophets, a prophet thinks he can use an elite for peace after noticing them trying to uncover artifacts and orbital bombardment?
