Access Override - Puzzles will not appear
[b]Vesper's Host Dungeon[/b]
For the "Access Override" secret triumph, the puzzles will NOT appear as long as you have a person on your fireteam that has the Ice Breaker Catalyst fully complete.
Once that person left our fireteam, the puzzles appeared and we were able to continue and finish all puzzles.
(We spent quite a while trying to figure out how to make the puzzles appear)
