This happened to me last night, too. Playing Exodus: Choral Mind solo, when I get to the choral mind, if I dye, when I respawn, I do so in the Ancillary Transit Nexus with no portals active. Even if I kill myself, I respawn in this area. I have to start the whole thing over again. Getting old.
