Don’t know if this is the one you’re not getting credit for, but I [i]believe[/i] the Keep of Honed Edges (which happens to be the current one in rotation) will only count as completed for the Triumph if you exit the portal while carrying a Hive Sword. Luckily the ability to repeatedly attack mid-air with the Hive Sword without losing much altitude makes it relatively easy to get back to the portal with on in-hand. (΄◉◞౪◟◉｀)
