A breakdown of the mistakes Bungie repeated, which they promised they would not repeat

Before House of Wolves came out, Luke Smith made a statement about how the biggest mistakes made in The Dark Below would not be repeated. At the time, this was true. House of Wolves rectified some of the biggest things like exotic resets, gear invalidation, and barriers to making gear usable.
I was thinking back on this article today and it occurred to me that just about every mistake they mentioned in that article was repeated in The Taken King.

You mean to tell me that you lessened the grind by making the level of your raid loot random, and by letting you level up fractions of a light point at a time?

An infusion system that requires you to pick out things to dismantle in a certain order in order to make one piece usable.

I know some of these things were addressed in the April update, and for that I'm grateful. Props to the team for making some of these much needed, much asked for improvements to the game. They really make the game playable. I appreciate the effort to make this game better.
But my point in this post is -- why does Bungie continue to make the same mistakes? Why do they wait so long to do something about them? The infusion system was called out for its bullshit not too long after the launch of TTK. There was talk of bad drop rates after the first week. What the actual -blam!-, man.

I'll be honest...
As a PvP player....
If they would just bring back rerolling and elemental primaries, I wouldn't even care.
(they would however need to massively nerf snipers.

As someone who enjoyed Y1...
Y2 was a downgrade. Yes, 3ofC made exotic farming easier, but due to the nerf galore, and the completely uncompelling King's Fall Loot table (now ignoring the subpar weapons, why the eff is cocoon a perk that takes an actually useful node away? Reloading is never a problem in this game....), I didn't bother a lot with that part of the end game.
