Praxic Blade & Matterspark
There is a minor bug when you go into Matterspark while having the Praxic Blade equipped where you can't get out of the form unless tim expires or you return to orbit. If going into Matterspark, switch off the Praxic Blade before doing so (jusy don't have it out).

To rule out any issues that may be coming from your controller settings, can you go to the Controller settings and try resetting to Default, to see if it helps?
