Kell fall spawn issue
At final boss fight on grandmaster if you die it spawns you outside boss room with no way to continue.
Happened once so I started another run.
Happened again.

Edit: no deaths at final boss fight and didnt want to test it out. Reply to the other poster explained on more detail about boss fight, it does seem random to spawn you outside of boss room during encounters. Sometimes it won't other times it will.
