Heavy attacks with the Ill Omen sword appear to inconsistently apply slow stacks via the Cold Steel perk. Sometimes no stacks are applied at all over the duration of the attack, sometimes multiple are applied at once, instantly freezing the target. Possibly applied more when it spawned a Radiolaria pool via the origin trait? Unsure. Behavior seems to be all over the place. Light attacks appear consistent though.
