When I claimed the "Become Naeem's Avatar, O Vengeance Mine" triumph I was awarded with the emblem Regicide, instead of Tattered Regalia. It seems like this is not a problem unique to me since the same bug happened to Esoterickk and my clan mates said the ship and ghost triumph are giving ghosts of the deep loot too.
