The “Deny Death” triumph is for completing the dungeon without dying or leaving the activity and if you’re in a two or three person fireteam then no one on said team can die in order to obtain the triumph. Then there’s “Alone in the Deep” which is for completing it solo without leaving the activity and “King of the Deep” which is for completing it solo flawless without leaving the activity. So like I said, if you’re in a team no one can die and your other two teammates did multiple times.
