Encountered this bug for [b]6[/b] times these two days, 4 happened while I was trying to do a solo flawless run, 2 happened when playing with my pals. Specifically, after cleaning up a level, you got a chance not lifting up to the next. Killing yourself is the only way to get out, but you know, I was doing a flawless :( Gotta say its ruinning my experience of the game so bad, especially when trying a challenge, hope Bungie can fix this soon.
