Which needs to leave. I had friends get a ghjallahorn or a vex mythoclast every time we did hard vog. I never got either once. They just scrapped them while I received legendary armor for a different class. Thats a major balance issue, especially with "G-horn or kick" lfg teams and the vex was the most op thing in existence when it was released. Terrible balance system. Also receiving useless items for endgame content like 3 shards for completing a nightfall or the same chest piece for the 100th time.

Its not some though.out of over 50 Hm VoG, never did I get a vex. Yet my friends got one 90% of the time. Thats not well balanced. The game should do built in 3oC, every time you don't get a certain exotic (make each raid boss have specific exotics, and strike bosses too) you get an increased chance of it. Each is randomly generated when you make a character or start a raid. Some people have a .0005 chance of ever dropping a Ghorn. Thats millions of years spent playing the game to get one exotic. Some people have a 100% chance of a Ghorn first time in a strike. Its not well balanced and is a cheap way to get people to play more. China made it law to show the percentage odds of getting exotics. It should be per boss %, not tied to your character. My main got the last word 3 nightfalls in a row. My two alts never got one.

Only to a point. You should be guaranteed certain high level loot, not every time of course, but not after 10000 hours either. Lets say for you there is a 2% drop of a vex. Each raid you so without getting it adds a increasing %. So +2%,+3%, etc... until the item drops for you. Once it does it is reset to a random # each time. Next time it might be a 95% and the next 10%. This ensures that even if it takes grinding it WILL drop the more times its completed. And high level content should not drop low tier items. There is a balance and Bungie did not do it correctly. Deej confirmed after launch that some players may never get certain drops without an unreasonable amount of playtime. Which is why Xur exists. I didn't recieve the suros or icebreaker until year 2 in VoG. But his inventory is controlled. He can't sell certain items. I'd like to see more "black spindle" type quests in D2.
