That's because someone hit the final key before your code was input go to Shiro with the other 2 players get new siva engines which are free then after everyone has the codes in then activate together

Same happened with us. In our case 1 guy got it. So if the guy who got it (outbreak prime) grabs the duplicate, does he need to go through the entire quest line to convert it from (stable) to (unstable) including the entire raid again? Or can he just pick it up from Shiro and we sync with him?
