The split second the Choral Mind died at the end of Encore, error code OLIVE and booted out to orbit. No quest completion for Act III, no loot, nothing. Troll level error, bravo Bungie. Looking up the error code just gives the generic: try it again, if that doesn't work, restart Destiny, it could be something on our end, blah blah... unhelpful.
