I filled out the form and never received a confirmation of receipt or anything. Still no emblem either :(

I went through this process and can save you some time. After contacting Bungie, they referred me to St. Jude's, who then referred me to a third party who was managing the distribution of the codes. I was ultimately able to get this resolved by emailing evidence of my donation (confirming timing and amount) to: gcx@raredrop.co I hope this helps!
