In first person games, the screen will randomly go dark for half a second to simulate your character blinking. Also activates if an attack barely misses to simulate flinching
Your character may or may not be lactose intolerant, in the situation you consume something with lactose, you'd have to find a toilet or suffer a debuff
You may stub your toe when walking past a chair or a table, if you do stub your toe, Be stunned for 3 seconds
After running for so long, your character will suffer from sore legs and will need to rest, if you neglect the sore legs, you'll suffer a stamina and movement speed debuff
Your character will get bored if you don't do anything fun, bored will reduce your social stats
Your character will not like certain foods and will not gain the full healing effects from foods if it is one your character doesn't like (yucky.)
You will have to go pee every 2 hours (or less if you drink water) and if you wet your pants, you will be embarrassed and suffer a major social stat debuff until you change your pants
These are probably stupid as heck but that's the point so :) feel free to add some more stuff

Your pants randomly fall down until you pay irl money for a digital belt. Literally anything will kill you if you play the game wrong. You must clean everything like someone with OCD to keep it in working condition.

You can catch a cold if you're not with good enough clothes in cold weather.

Allergies! Your character can have randomly determined allergies to certain things of varying degrees. For example, your character could be mildly allergic to bird feathers and every time you are around then your eyes redden giving some minor debuff. Or perhaps your character has a major allergy to a type of cheese, and eating said cheese might just kill your character. I have done something like this before in D&D games as a joke. Random players of mine where given minor allergies, unbeknownst to the them, to certain monsters. For example, I describe one of my players characters having a runny noise every time gnolls are nearby.

Didn’t Vice City have an ankle roll mechanic? I know you’d spontaneously take a tiny amount of damage unpredictably when running off and on the sidewalk. I remember being killed by that more than once.

Flash bangs in a VR shooter make your screen white and play an ear splitting noise to simulate a real flash bang.

I was playing Diablo IV and making a new character. The thought of “roll for disability” came to mind. Or if you were wearing an eye patch, half your screen is black. A 1 in 100 chance to randomly choke on your spit. Particle effects have a chance to make your character sneeze.

The food one is actually a thing in Crystal Chronicles. Your tastes change if you eat enough of the food. Each food also gives buffs (like defense or strength), so you can set your tastes to like the food with your preferred buffs. Food preferences don’t affect the buffs, only the heals. I actually kind of dig it. Another fun aspect of the build to customize. Unfortunately, CC actully has a bit of a healing problem, so it means very little in the long run. Lilties are basicallty the only class that uses food instead of healing spells.
