Not how it works. Once you are reported, and amass alot of reports, bungie will pull the traffic logs of your matches.. If they see packetloss or poor ping, they will issue a restriction. That gives you a chance to sure up your connection, before the restriction is up

It may be 0 when the test is performed. During gameplay, it may be very intermittent and slight, but enough for others to notice and report what they are seeing

Ya. I would make sure everything is reset, make sure the hardwired connection is good. Also if going into pvp, make sure no one is dowloading anything or eating up your bandwidth
