Spire of the Watcher Solo flawless
I just did a spire of the Watcher Solo flawless, more de 4 hours into the run, and guess what?! I didn't get the triumph! bc somehow, a error that made me respawn without any ammo, but alive counted as a death. Congratulations Bungie! Apparently dungeon.report is more efficient at tracking peoples achievements than Bungie.

Unfortunately you will have to do the dungeon again from the start to get the triumph. Note that the API from Bungie does not provide third party developers all data and third parties can also collect and show varying degrees of information that are less than the full picture.
