Prophecy flawless bug
I just completed the dungeon without dying, got the banner at the bottom of the screen for the flawless run, but the triumph hasn't been completed and I haven't been able to claim the reward.
my game history even shows no deaths and I played in one sitting.

Hey there! You did go flawless, however, one fireteam member has a less time than you and your other teammate. I am not exactly sure if having someone join the fireteam late affects this triumph. Either way, something has prevented you from obtaining the flawless triumph. You would just need to do it again and make sure everyone joins together.
