Just lost a weapon to the app transfer
Just sitting on venus and used the app to get my bad juju out of the vault and poof gone. Wasn't in a cutscene, orbit or activity except just patrol. Reset my xbox and nope still gone. I'm pretty pissed and not sure what to do. My feedback is use this feature at your own risk.

It will help the folks at Bungie investigate into a fix. I'm sorry to hear that your Bad Juju went missing, anecdotally some users have reported that their missing items will randomly show back up, I hope that this will be the case for you too.

Lost one exotic shard during a transfer. Was prepping for crota while some team mates waiting on hard mode. I spawned in when I transferred (thankfully) one of five exotic shards. That shard was gone forever. So if in orbit or loading, it seem to work. But getting a weapon while dead, it's gone. I'm glad it wasn't a weapon. Hope this gets seen.

Check your vault. This happened to be and it shot it to my vault. Also check your other characters

Had the same issue with Hezens Vengeance RL. Did a transfer and it vanished was pretty pissed but it eventually showed back up in my vault.

Same thing happen to me a while back, I tried transferring my Red Death from my vault and when I went to equip it... Poof it wasn't anywhere to be seen. Then I realized that this game still hasn't -blam!-ing given it to me and then I woke up. True story

Ouch, sorry to hear. I really hope this doesn't happen to my ghallarhorn. I might quit destiny if it does.
