Once you log in, you will be taken to the Customer Support page. For your Ghost Edition box, select Hardware and search "Destiny" to find the Ghost Edition selection. Follow the steps provided, and describe your issue in detail. Activision will require your contact information, so make sure you key it in correctly and be ready to provide any additional information they may require.  

NOTE: You must have an Activision account, or create an account if you do not have one. Activision Customer Support can only help with warranty and disc/goods replacement and cannot provide game support. All game support is provided on
