They should just add a kill boundary to every location they don't want the player to be in.
Raid bosses being knocked off, easy fix. Make the loot physical and if the boss falls off, the loot goes with it. Punish players for avoiding the fight by not rewarding them with anything.
Update major enemies to flank the player if they're hiding for too long.
Put all starter safe zones before the boss area out of bounds as soon as a player engages the boss. This gives the player an opportunity to clean the room and then work on the boss without any adds until drop ships or waves start coming in.
