Over the last few weeks I've noticed what appears to be a bug with my Warlock's healing rift. Sometimes I'll hit the key to activate it and nothing happens. The camera zooms out and my character does the casting animation, but nothing is placed on the ground, no healing is triggered, and rift-related abilities (like Arc and Void Souls) don't proc. This has happened with both Arc and Void subclasses, and in both Crucible and Portal activities.
