This just happened to me and I'm still laughing from it. If you pet the frog while someone is talking to you on the radio, failsafe will interrupt them to say "Good frog!" and then the radio message will stop with no way to replay it. Why put something there you know players are going to interact with, especially during the boring radio messages, that can break said radio messages? Did this not happen in testing?

I'd rather listen to Failsafe for 2 seconds saying "good frog" , than listen to Saint and Osiris drone on about their love issues. This is a good thing.

Sooooo many little things that you'd imagine would be super easily caught if they had 3 people do new activities make it through. Like this one, you'd think somebody would've tried it during testing. And.... they probably would've.

If you fast travel back to Failsafe you can start the message over from the beginning to progress the Triumph. (΄◉◞౪◟◉`)
