I had this happen recently when I ran Salvation’s Edge. In Verity, in normal circumstances, the wall in the back of the room changes shapes according to what shapes you have currently. Even if you have 2 of the same shape, it’s supposed to shift.
Someone in my fireteam died during the encounter and I got transferred into isolation as is the encounter’s design. There, I experienced a bug where my wall was stuck on square the whole time. Not even rotating or shifting. It was just stuck for whatever reason. Even when the intended wipe mechanic happens when you get saved, it didn’t resolve the problem.
End Result: My fireteam wiped because I could not see what shapes I had. The wall during that attempt was useless since it glitched.
I have a video tagged in this post showing what I saw. Pay attention to the wall in the back of the room that supposed to change shapes.
