have you un-installed, rebooted and cleared the cache (google it) and re-installed? it's possible the drive you have it installed on in your ps4 is developing some bad sectors.

I'm having the same problem and it seems to be getting worse. I've never had a issue like this before. It started shortly after season of opulence update. I've tried everything to fix it from cache dump to uninstalling and reinstalling game. Very frustrating
