In Rheasilvia, at the location for the locked chest (the one where you need 3 arc charges to unlock it), If you look east you can see a ledge (sometimes a random chest spawns there). Jump to the ledge, to your left there should be 2 trees, jump to the top one, right over that there is a hole in the cliff that looks like it leads to the surface with sunlight, but it is a hole that leads down under the surface texture.

Holes like that are all over the place, pretty much every map in the game has at least one OoB hole somewhere. I'd add something about poor level design, but it's actually fun playing around with that kind of stuff every once and a while.

In the Dreaming City, in the Harbinger's Seclude, there's a spot where you can jump outside the map, which gives you access to an insanely large area. It's all empty, but it's pretty fascinating to explore nonetheless. Edit: And there's a pretty huge area you can get to on Io too.
